What forms do I need for my son/daughter to participate in sports at
You will find links to all of the paperwork necessary for your child to participate in school sports below.
In order to complete the paperwork properly, please note the following:
· The Assumption of Risk form must be notarized to be valid.
· A student only needs 1 Assumption of Risk form filled out during his/her entire middle school career.
· Please check with your coach regarding your Assumption of Risk form; he/she will have a current student list.
· Students need a current physical exam (PE) to participate.
o A current PE is less than 1 year old during the season.
· If the PE expires during the season (is more than a year old) the student has to either turn in another PE or get written permission from his/her doctor to continue to participate.
· The coach will submit a list to the nurse to confirm athletes with current physicals.
· Students need a current PE to even tryout for a sport
· The Pre-Participation form can be filled out by a parent.
· The insurance information on the Sports Permission form must include a policy number.
