2) Long Term-Medication Form: Any student taking prescription medication, for any reason, must have the following form /permission slip signed
by his/her doctor and parent/guardian. Please click here to download the Long-Term Medication Form.
3)Immunization Requirements for All Children Entering Grade 7: In accordance with the Rhode Island Department of
Health Rules and Regulations Pertaining to Immunization and Testing for Communicable Diseases (R23-1-IMM), all children entering the 7th grade are required to
have the following immunizations:

*Four (4) doses of Polio vaccine
*Two (2) doses of MMR vaccine (Measles, Mumps, Rubella)
*Three (3) doses of Hepatitis B vaccine.
*Two (2) doses of Varicella (chickenpox) vaccine received or a statement signed by your child’s doctor stating that your child has a history of
chickenpox disease.
*One (1) dose of Meningococcal conjugate (Meningitis) vaccine
All children entering 7th grade are required to be up-to-date on their immunizations as well as have a physical exam. This is the perfect opportunity to review
your child’s immunizations with the doctor to ensure your child is protected from all vaccine-preventable diseases.
Important Reminder ABOUT FLU VACCINATION: An annual Influenza (flu) vaccination is now recommended for all children 6 months-
18 years of age to prevent the spread of flu to others in daycare, preschool and school settings as well as at home and in the community. Influenza vaccine is
available from October through April each year. Don’t forget to ask your doctor about flu vaccine
4) Health Screenings: The health and well-being of students is very important and plays a significant role in academic achievement and success. To assure
that your child has the best possible opportunity for learning, the Cumberland School Health Services Department conducts the following health screening each
year, as mandated by the Rhode Island Departments of Health and Education.
Vision Screening: All 7th grade students – done by the school nurse/teacher.
Dental Examination: 7th grade students only – done by the school dentists, Dr. Johnson
and/or Ward or a qualified dental hygienist under their supervision. Only those students in grade 7 who have not returned a yellow dental card will be examined.
Scoliosis Screening: All students in grades 6, 7, and 8 will be screened by the school nurse/teacher.
Physical Exam: New students and all students in grade 7 who have not returned the required physical examination form will be examined by the
school physician, Dr. E. James Monti. You will be notified before the examination.
Please be advised that if your child plans on participating in any school sports, he or she is required to have a current physical exam on file with the nurse. A
current physical is one that is dated less than one year before the last day of your child’s sports participation. You will be promptly notified of any health
problems detected during these screenings. Screenings are conducted at various, still to be scheduled times throughout the school year. If you do not
want your child to be screened at school, you must return the appropriate forms completed by the child’s physician or dentist and notify the school
nurse/teacher, in writing, as soon as possible. Without the required documentation, we will assume that your child is to be screened at school.
